Please arrive to your appointment 15 minutes early with Insurance Card, Driver’s License, and Work ID (healthcare, essential works). If you show up more than 15 minutes after your dedicated appointment time, we reserve the right to reschedule you for a later date.
Per the New York State COVID-19 phased distribution schedule, we are currently ONLY ABLE TO OFFER THE VACCINE TO THE FOLLOWING GROUPS:
Medical professionals and their office staff, home health care workers, teachers and education workers, first responders, public safety workers, public transit workers, people age 65 and older. For a more detailed list, please CLICK HERE. We will require a work ID for all healthcare and essential workers to confirm their eligibility. In addition, please also bring your driver’s license and insurance card to your appointment.
The vaccine is only approved for those 18 years and older, NO exceptions.
CURE Urgent Care is currently offering the Moderna vaccine. More information about the Moderna vaccine may be found HERE.
The vaccine may have mild side effects, including but not limited to: Pain at the injection site (upper arm), fatigue, headache, chills, fever, and joint and muscle pain. These side effects should be mild and short-term. If symptoms persist or additional symptoms arrive, please reach your medical provider immediately.
Yes, your second dose will be scheduled for you at the time of your first appointment. You will receive your vaccine certification card at the time you receive your first vaccine. You will need to bring the certification card back with you to receive your second vaccine.
NOTE: There is a 15-minute mandatory observation period after you receive your vaccine, so please budget your time accordingly.
There will be no out-of-pocket cost to you for the vaccine. If you are insured, please bring your insurance card with you on the day of your appointment. If you are uninsured, you will pay nothing for the vaccine.
